Care Coordinator

About the role

The Care Coordinator is responsible for participating with the therapeutic team while supporting and reinforcing treatment services by providing guidance and direction to assigned individuals, families, and groups, and coordinating internal and external resources to ensure ongoing therapeutic services and community support is delivered for complete treatment objectives for stabilization achievement.

Responsibilities

• Provide mental health services/counseling services to assigned youth and document service delivery to meet or exceed minimum standards.
• Manage and coordinate resources and activities for each assigned client to ensure unique treatment needs and requirements are met.
• Coordinate, provide transportation, and facilitate/co-facilitate case conferences and client-oriented team meetings with internal and external resources, including county or state referral sources, court appearances, and scheduled family visits.
• Actively participate in the implementation of individual treatment plans, compile resident assessments, maintain required clinical documentation, and meet provided deadlines.
• Collaborate with and maintain regular contact with family members, licensing, parole officers, the court system, and other organizations to ensure consensus on therapeutic treatment direction, behavior expectations, progress, and outcomes.
• Maintain individual accounts and residential records, ensuring basic needs of clients are met (e.g., adequate clothing) and engage youth in managing their own budgets.
• Maintain a flexible schedule to meet changing work needs and demands of clients and their families.
• Facilitate the client and family’s linkage to the therapeutic treatment process, identify barriers, and connect with support throughout treatment to aid post-transition or discharge placement.
• Work closely with teams to create supported engagement, guidance plans, stabilization plans, treatment plans, therapeutic interventions, and discharge plans.
• Participate as an active member of an interdisciplinary team to develop treatment plans and complete other client or case documents as needed.
• Assist families in locating resources to support the healthy functioning of the family unit and bridge relationships with outside organizations.
• Participate in and/or facilitate all required meetings and conferences relative to assigned clients.
• Maintain and complete paperwork and documentation required by the organization and governing/accrediting agencies; keep current files on all individuals enrolled in services.
• Provide consultation and mental health services to youth, families, and support systems, documenting services consistent with Medicaid requirements to facilitate the treatment process.
• Ensure completion of all required documentation standards, including critical incident reporting, and produce reports as requested by outside agencies, parents, or courts.
• Work effectively with internal stakeholders to implement plans for achieving treatment objectives and community reintegration.
• Participate in the training and development of other care coordinators and assume a mentorship role as assigned.
• Maintain appropriate boundaries in all interactions and contact with residents.
• Comply with Nexus policies involving privacy and confidentiality for residents and their families.
• Handle emergency or crisis situations with sound judgment and appropriate actions.
• Remain calm and effectively manage stressful workload conditions and emotionally charged or crisis situations.
• Demonstrate concern and empathy while maintaining appropriate boundaries.
• Exhibit a high degree of ethical conduct, integrity, and confidentiality, adhering to the Nexus Code of Conduct.
• Demonstrate self-awareness and willingness to process emotional and behavioral responses to workplace situations, using professional judgment in daily work.
• Recognize and value cultural differences in all aspects of work and service delivery.

Requirements

• Bachelor’s degree in Social Work preferred; Bachelor’s in Human Service Degree required.
• Minimum 21 years of age.
• Must successfully pass a DHS/DCFS/CJIS/DHHS/ODHS or other federal and state criminal background check.
• Valid driver’s license required; must meet state regulating agency driving requirements, maintain auto insurance, and pass corporate insurance eligibility standards.
• Ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
• Openness to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.

Preferred Qualifications

• Previous experience working with youth.
• Previous experience working in a residential setting.

Physical Demands

• Ability to assist staff in physical interventions and restraints of clients.
• Ability to lift up to 50 pounds unassisted and up to 100 pounds with assistance.
• Ability to stand and walk for extended periods of time (2-4 hours).
• Ability to twist, bend, and stretch in a manner conducive to daily activities within a residential treatment facility.
• All client-facing Nexus employees must successfully participate in and complete a Handle With Care behavior intervention method exam and training.

Pay

$24-$26 hourly

Schedule

Monday - Friday, on-site at Luther Hall facility, Fargo, ND.
