Job Description
This position reports to the Director of Operations and works on-site at 6019 S 66th E Ave, Suite 200, Tulsa, OK 74145.
About the role
Care Coordinators help match clients and caregivers, acting as a case manager for ongoing needs. They build relationships with clients and caregivers, complete monthly check-ins, and participate in on-call duties as needed.
Responsibilities
- Buils relationships with clients and their families to match them with caregivers who can provide the needed services.
- Builds relationships with caregivers to match them with clients and assignments that fit their needs and improve retention.
- Completes monthly check-ins with clients, ensuring satisfaction with the services provided. Check-ins include text, email, phone calls, and in-person visits.
- Creates a positive, thorough, and welcoming onboarding experience for new clients.
- Keeps scheduling system up to date and accurate with client schedules, contact information, and preferences.
- Participates in on-call duties on a rotating basis and as needed by the office.
- Works closely with the Talent Specialist to create a successful, supportive, and ongoing experience for clients and caregivers.
Requirements
- Ace or related field preferred but not required.
- Previous experience in the home care industry, human services, or a similar field required (or a strong desire to help others).
- Ability to pass all hiring criteria as defined by Griswold standards and/or state regulations.
- Reliable transportation, valid driver's license, and valid car insurance.
- Knowledge of standard business equipment including a laptop computer and a mobile phone.
- Ability to see and interpret information within Griswold systems and exchange information accurately and quickly.
- Ability to lift up to 40 pounds and alternate between stationary and traverse positions.
- Strong verbal and written communication skills.
- High level of emotional intelligence and empathy, as well as dedication to core pillars.
- Creative problem-solving skills.
- Organized and detail-oriented.
- Flexible and able to multi-task.
- Effective time management skills.
- Proactive with strong initiative and follow-through.

- Associate degree in human services or related field preferred but not required.
