About the role
Full-time Care Coordinator in a Birth to Three Program supporting children aged 0–3 with developmental delays or disabilities, and their families.
Responsibilities
- Conduct assessments
- Develop and implement family-centered service plans
- Coordinate services
- Support families through education and advocacy
- Facilitate team meetings
- Maintain documentation
- Collaborate with internal teams and community providers
The role involves working in home, office, and community settings, with flexible scheduling based on client needs.
Qualifications
- Bachelor’s degree in Social Work, Child Development, or related field
- Preferred experience working with families with special needs
- Valid driver’s license and reliable transportation required
- Strong communication, organizational, and documentation skills
- Ability to handle crisis situations
Work Environment
- Community-based with daily travel
