Patient Coordination & Support
Serve as a consistent resource for patients and providers, addressing questions and concerns promptly. Directly schedule and facilitate follow-up appointments within HSS and with external providers, ensuring full closure of the referral loop. Perform pre-appointment screening to confirm program appropriateness and redirect when necessary. Provide patients with letters, prescription renewals, and instructions per provider direction.
Monitor and manage patient work queues; perform check-in/check-out procedures; ensure timely completion and closure of encounters. Obtain and upload prior authorizations, test results, and related documentation into patient charts for provider review.
Answer inbound calls and conduct outbound follow-up communications to ensure timely next steps.

Operational Responsibilities
Collaborate with care navigation teams, physician practices, radiology, PT, and ancillary services to streamline care delivery. Build and maintain professional relationships across all service lines to improve care coordination and patient satisfaction.
Qualifications
- Education – Required Associate’s degree required; Bachelor’s degree preferred. In lieu of education, 5–7 years of relevant experience may be considered.
- Experience – Required Minimum 2 years of experience in a patient-facing outpatient role.
- Skills & Competencies Superior communication and customer service skills, with the ability to handle patient or provider frustration professionally. Strong organizational and problem-solving abilities with independent decision-making skills in a fast-paced environment. Demonstrated reliability in following assignments to completion and prioritizing patient care needs. Proficiency with Microsoft Office, Epic, and Salesforce; strong computer literacy and internet navigation skills. Familiarity with medical terminology, medical billing, and insurance processes. Quick learner with the ability to understand and convey new information effectively.

The salary of the finalist selected for this role will be determined based on various factors, including but not limited to: scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation.
Pay
The base pay scale for this position is $41.38 - $46.04.
