About the role
We serve communities across multiple states with a high-touch, people-centered care model. Our teams are carefully selected and developed to ensure they have the skills and support needed to deliver exceptional care.
Responsibilities
- Provide compassionate and skilled end-of-life care to patients and their families.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
- Ensure compliance with all regulatory requirements and standards of care.
- Engage in ongoing professional development to enhance patient care and team effectiveness.
Requirements
- A bachelor's degree in a healthcare-related field or equivalent experience.
- At least 2 years of relevant experience in hospice or palliative care.
- Current certification in Basic Life Support (BLS).
- Strong interpersonal and communication skills.
- Ability to work flexible hours, including evenings and weekends.
