Responsibilities

• Provide compassionate, skilled care to patients and their families
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
• Document patient care activities accurately and timely
• Adhere to all regulatory requirements and policies

Requirements

• Bachelor's degree in a healthcare-related field
• Current license to practice in the state(s) where you will be providing care
• Experience in hospice or palliative care preferred
• Strong communication and interpersonal skills
• Ability to work flexible hours including evenings and weekends
