Responsibilities

• Provide comprehensive primary healthcare: Partner with physicians to deliver personalized care by diagnosing and treating acute and chronic conditions to support long-term patient wellness.
• Deliver high-quality clinical assessments: Perform thorough patient examinations, interpret diagnostic studies, and create effective treatment plans to ensure the best possible health outcomes.
• Manage complex care journeys: Coordinate seamless treatment pathways by collaborating with multidisciplinary teams and facilitating referrals to appropriate community resources.
• Empower patients and families: Provide clear, accessible education on preventative health, treatment options, and community resources to support informed decision-making.
• Maintain clinical excellence: Document patient interactions and care processes with precision, ensuring consistent adherence to high-quality standards and organizational policy.

Qualifications

Licensure / Certification / Registration: One or more of the following required:

• Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.

Preferred Credential(s):

• BLS Provider. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration.

Education: Master's degree of Nursing required.

Skills

• Experience preferred, but will train the right candidate.

Pay

• Start Bonus: $5,000
• Productivity incentives: RVU
• CME allowance: $2,000
• Relocation Allowance: $10,000
