Key Responsibilities

  • Serve as a trusted advocate for clients, ensuring they receive the highest standard of care and support.
  • Build and maintain strong relationships with healthcare providers and referral sources.
  • Care Coordination & Quality Assurance:
    • Oversee client service plans, ensuring that care is adapted as needs evolve.
    • Collaborate with care team, schedulers, and care pros to ensure seamless service delivery.
    • Conduct regular quality assurance visits and update documentation accordingly.
    • Lead client and care pro introductions, ensuring a smooth and positive experience for all.
  • Collaboration & Problem Solving:
    • Address client concerns with empathy, professionalism, and proactive problem solving.
    • Work closely with care pros, health professionals, and family members to create the best outcomes for clients.
    • Identify opportunities to enhance care services and advocate for additional support when needed.

Qualifications & Experience

  • Prior experiences in client services, healthcare, senior care, social work, or care coordination strongly preferred.
  • Experience working directly with older adults, families, and caregivers is a plus.
  • Strong networking, presentation, and relationship building skills.
  • Proficiency in Microsoft Office Suite and CRM software (Salesforce).
  • Ability to operate independently and manage a high level of field-based activity.
  • Valid driver’s license and reliable transportation required.
