Key Responsibilities
- Serve as a trusted advocate for clients, ensuring they receive the highest standard of care and support.
- Build and maintain strong relationships with healthcare providers and referral sources.
- Care Coordination & Quality Assurance:
- Oversee client service plans, ensuring that care is adapted as needs evolve.
- Collaborate with care team, schedulers, and care pros to ensure seamless service delivery.
- Conduct regular quality assurance visits and update documentation accordingly.
- Lead client and care pro introductions, ensuring a smooth and positive experience for all.
- Collaboration & Problem Solving:
- Address client concerns with empathy, professionalism, and proactive problem solving.
- Work closely with care pros, health professionals, and family members to create the best outcomes for clients.
- Identify opportunities to enhance care services and advocate for additional support when needed.

Qualifications & Experience
- Prior experiences in client services, healthcare, senior care, social work, or care coordination strongly preferred.
- Experience working directly with older adults, families, and caregivers is a plus.
- Strong networking, presentation, and relationship building skills.
- Proficiency in Microsoft Office Suite and CRM software (Salesforce).
- Ability to operate independently and manage a high level of field-based activity.
- Valid driver’s license and reliable transportation required.
