Birth Certificate Clerk - Medical Records
Job Summary
The Birth Certificate Clerk works closely with the Physicians, Nurses and OB Staff to identify all deliveries to ensure an accurate timely birth certificate is prepared for all births within (3) days of delivery. Accurately enters all required data into the state EBRS- Electronic Birth Registration System. Identifies and completes a Paternity Affidavit as applicable.
