About the role
This virtual role enables associates to work remotely full-time, with the exception of required in-person training sessions. Candidates must reside within a commuting distance from an office location or be granted an accommodation as required by law.
Schedule
Monday–Friday, 8:00 AM–5:00 PM, plus rotating on-call shifts.
Responsibilities
- Provide urgent health care via telephone and tele-video to patients receiving home and community-based services through state Medicaid programs, dual-eligible members, and other assigned membership.
- Develop and implement clinical plans of care for adult patients with chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions).
- Gather history, perform physical exams, order diagnostics as needed, and create treatment plans aligned with patient goals and current conditions.
- Identify and close gaps in care.
- Address patients’ and families’ physical and psychosocial needs with support from the interdisciplinary team.
- Educate patients and families on medication usage, side effects, illness progression, diet, nutrition, medical adherence, and crisis anticipation and prevention.
- Maintain contact with clinical team members and other medical providers to coordinate optimal care and resources in a timely manner, consistent with state regulations and company standards.
- Maintain accurate patient medical records and documentation per state regulations and company policy.
- Participate in continuing education as required by state and certifying bodies.
- Prescribe medication as permitted by state prescribing authority.
Requirements
- Master of Science in Nursing (MSN).
- Active national Nurse Practitioner (NP) certification.
- Valid, current, active, and unrestricted Family or Adult Nurse Practitioner (NP) license in Texas, Kansas, or Iowa.
- Active, unrestricted RN Compact license.
- Bilingual or multi-language skills required.
- Minimum of 2 years’ experience managing complex care cases.
- Experience working with Electronic Medical Records (EMR).
Preferred Qualifications
- Medicaid provider number.
- DEA registration or eligibility.
- Experience managing complex care cases for developmental disabilities and chronically ill patients.
