Nurse Practitioner, Bilingual (100% Virtual) - CareBridge

About the role

The Advance Practice Provider, Nurse Practitioner Bilingual role at CareBridge Health is seeking bilingual Nurse Practitioners licensed in Massachusetts. Candidates must have an active RN license and experience managing complex care cases. The role is virtual, enabling associates to work full-time with occasional in-person training sessions.

Responsibilities

• Collaborate with company physicians, other providers, and patients' families to develop complex care plans.
• Provide primary and urgent health care via telephone and tele-video modalities to patients receiving home and community-based services through Medicaid programs and other memberships.
• Develop and implement clinical plans of care for adult patients with chronic and complex conditions.
• Gather patient history, perform physical exams, and diagnostics as needed, then develop and implement treatment plans based on patient goals and conditions.
• Identify and address gaps in care, meet patients' and families' physical and psychosocial needs, and educate them on medication usage, illness management, and crisis prevention.
• Maintain contact with other clinical team members, other physicians, and medical providers to ensure coordinated care and resource allocation.
• Manage patient medical records and documentation according to state regulations and company standards.
• Participate in continuing education as required by state and certifying bodies.
• Prescribe medication as permitted by state prescribing authority.

Requirements

• Master’s degree in Nursing.
• Active national NP certification.
• Active, unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Massachusetts.
• Valid, current, active RN license.
• Experience managing complex care cases for adults.
• Bilingual or multilingual skills in Spanish.
• Experience with Electronic Medical Records (EMR).

Preferred Skills, Capabilities and Experiences

• Active RN Compact license.
• Experience with developmental disabilities and chronically ill patients.
• DEA registration or eligibility.
• Bilingual in multiple languages, particularly Spanish.
• Active Medicaid number in Massachusetts.

Pay

$126,592 to $158,240 annually.

Schedule

Monday - Friday, 8:00 AM to 5:00 PM EST, with rotating on-call shifts from 5:00 PM to 8:00 AM EST.
