Nurse Practitioner, Bilingual (100% Virtual) - CareBridge
About the role
CareBridge Health is seeking bilingual Nurse Practitioners licensed in the state of Massachusetts. Candidates must have an active RN license and experience managing complex care cases.
Responsibilities
- Collaborate with company physicians, other providers, and patients' families to develop complex care plans.
- Provide primary and urgent health care via telephone and tele-video modalities to patients receiving home and community-based services.
- Develop and implement clinical plans of care for adult patients with chronic and complex conditions.
- Gather patient history and perform physical exams and diagnostics as needed.
- Identify and address gaps in care, meet patients' physical and psychosocial needs, and educate them on medication usage, disease management, and crisis prevention.
- Maintain contact with other clinical team members, coordinate care, and ensure compliance with state regulations and company standards.
- Maintain patient medical records and documentation.
- Participate in continuing education as required.
- Prescribe medication as permitted by state authority.
Requirements
- Master's degree in Nursing.
- Active national NP certification.
- Valid, current, active, and unrestricted Family or Adult Nurse Practitioner (NP) license in Massachusetts.
- Valid, current, active, RN license.
- Experience working with Electronic Medical Records (EMR).
- At least 2 years of experience managing complex care cases.
- Bilingual or multilingual skills in Spanish.
Preferred Skills, Capabilities & Experiences
- Active RN Compact license.
- Experience managing complex care cases for developmental disabilities and chronically ill patients.
- Possession of DEA registration or eligibility.
- Bilingual in Spanish.
- Active Medicaid number in Massachusetts.

The salary range for this specific position is $126,592 to $158,240, with actual compensation varying based on location, experience, and performance.
Schedule
This role is full-time, with a work shift of Monday to Friday, 8:00 AM to 5:00 PM EST, and rotating on-call shifts from 5:00 PM to 8:00 AM EST.
