Nurse Practitioner 100% Virtual, CareBridge

Seeking Nurse Practitioners licensed in either Ohio or New Jersey and holding an active RN Compact License. Residency in either state is not required.

About the role

This virtual role enables full-time remote work, except for required in-person training sessions. Alternate locations may be considered if candidates reside within commuting distance from an office. Candidates not within a reasonable commuting distance from the posting location(s) will not be considered unless an accommodation is granted as required by law.

Schedule

• Monday – Friday, 8:00 am to 5:00 pm EST
• Rotating on-call 2-3 days per month from 5:00 pm to 8:00 am EST

Responsibilities

• Provides urgent health care via telephone and tele-video modalities to patients receiving home and community-based services through state Medicaid programs, dual eligible members, and other assigned membership.
• Develops and implements clinical plans of care for adult patients with chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions).
• Gathers history, performs physical exams, and orders diagnostics as needed to develop and implement treatment plans aligned with patient goals and current conditions.
• Identifies and closes gaps in care.
• Addresses patients’ and families’ physical and psychosocial needs with support from the interdisciplinary team.
• Educates patients and families on medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention.
• Maintains contact with clinical team members and other medical providers to coordinate optimal care and resources in a timely manner, consistent with state regulations and company standards.
• Maintains accurate patient medical records and documentation per state regulations and company policy.
• Participates in continuing education as required by state and certifying bodies.
• Prescribes medication as permitted by state prescribing authority.

Requirements

• Master of Science in Nursing (MSN).
• Active, national Nurse Practitioner (NP) certification.
• Valid, current, and unrestricted Family or Adult Nurse Practitioner (NP) license in Ohio and/or New Jersey.
• Valid, current, and active RN Compact License.
• 2+ years of experience managing complex care cases.
• Experience working with Electronic Medical Records (EMR).

Preferred Qualifications

• Active RN Compact license.
• DEA registration or eligibility.
• Active Medicaid number in Ohio and/or New Jersey.
• Experience managing complex care cases for developmental disabilities and chronically ill patients.

Pay

For candidates working in Columbus, OH, Cleveland, OH, or New Jersey, the salary range for this position is $104,576 to $189,888. The salary offered is based on legitimate, non-discriminatory factors such as geographic location, work experience, education, and skill level.
