About the Role

CareBridge Health, a proud member of the Elevance Health family of companies within the Carelon business, exists to enable individuals in home and community-based settings to maximize their health, independence, and quality of life through home-care and community-based services. We are seeking experienced Nurse Practitioner candidates who hold an active, unrestricted Family or Adult Nurse Practitioner license in the State of Texas and a current, active RN Compact license.

This is a virtual role that enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within commuting distance from an office. Per our hybrid/virtual work policy, candidates not within a reasonable commuting distance from the posting location(s) will not be considered unless accommodation is granted as required by law.
