About the role
The Lead Medical Coordinator oversees health services and coordinates timely, appropriate health services for children in care. They also supervise Medical Coordinators and provide training on relevant medical, dental, and public health topics.
Responsibilities
- Perform all work consistent with NYAP’s mission, values, and philosophies.
- Ensure confidentiality of health records and adhere to NYAP, State, and Funder guidelines for PHI.
- Communicate health needs to case managers and clinicians.
- Schedule and transport children to/from health care appointments.
- Aid in obtaining medical clearances from healthcare providers.
- Prepare and follow up on Treatment Authorization Requests (TARs).
- Ensure qualified interpretation and translation services are in place.
- Collaborate with children’s assigned cross-functional team and medical, mental health, dental, and pharmacy providers.
- Elevate medical concerns to supervisors, healthcare providers, and the Division of Health for consultation and guidance.
- Support foster parents by providing education on medical policies, answering questions, and supplying necessary documents.
- Collect and upload forms and health records from medical providers.
- Maintain medicine cabinets and first aid kits.
- Provide in-service training and ongoing education to staff and foster parents.
- Assist Program Leadership during auditing processes.
Requirements
- Licensed Registered Nurse (RN) with a Bachelor's or Higher health-related education qualification.
- Bilingual fluency in English and Spanish is required.
- Minimum of 1 year of employment in public health or a healthcare setting with experience in care coordination, health administration, or direct clinical care.
- At least 1 year of experience working in the child welfare field with children and/or adolescents in a social service setting.
- Valid driver’s license, excellent driving record, safe and clean personal vehicle, and current automobile insurance.
- Proficient use of computers, smartphones, tablets, and software.
- Experience scheduling medical appointments and documenting medical appointments.
- Recognize and understand the needs of the immigrant population and create a safe environment for the children in the program.
- Demonstrated ability to maintain liaisons with community health providers and expand the community health network.
- Willingness to work non-traditional hours and be available for on-call phone consultations.
Qualifications
- Physical demands include use of manual dexterity, tactile, visual, and audio acuity; use of repetitive motion, prolonged sitting and standing, and sustained visual and mental applications and demands; occasional lifting (up to 25 pounds); bending, pulling, and carrying; ability to travel frequently and drive for extended periods.
