BILINGUAL PATIENT CARE COORDINATOR

About the Role

The Patient Care Coordinator organizes and provides support services for patients with individualized needs outside of the clinic’s scope of services under the supervision of the Operations Manager. Responsibilities include Patient Medication Assistance, Referral Care Coordination, Maternal Care Coordination, and general Case Management. All activities are documented in the Electronic Health Systems (EHS) for reporting and provider review as outlined in the policy and procedures manual.

Responsibilities

• Coordinate patient medication assistance programs.
• Facilitate referral care coordination for specialized services.
• Provide maternal care coordination support.
• Perform general case management duties.
• Document all activities in the Electronic Health Systems (EHS).

Requirements

• High School Diploma or equivalent.
• One year of healthcare experience in patient registration, admissions, or billing preferred.
• Medical Assistant training preferred.

Physical and Mental Requirements

• Sits, stands, walks, and converses for most of the day.
• Ability to operate general office equipment including telephone, calculator, computer, and printer/fax.
• Exerts up to 10 pounds of force occasionally (pushing, lifting, carrying, and/or pulling objects).
• OSHA Category III: No exposure to blood, body fluid, or tissue; no requirement to perform or assist in emergency care or first aid.
