Bilingual Case Manager/QBHS
Community Health Alliance is seeking a Bilingual Case Manager/QBHS who is fluent in both Spanish and English to provide culturally responsive, community-based behavioral health services and support to Spanish-speaking children, adolescents, adults, and families. This role is community based and includes home visits, service coordination, and collaboration with providers, families, and other supports.

What You'll Do
- Coordinates assessments, treatment planning, or crisis intervention services.
- Facilitates healthcare, behavioral health, and other services and supports as part of care coordination.
- Provides training and facilitates linkages for the use of community resources.
- Monitors symptoms and service delivery to ensure client needs are met.
- Obtains or assists clients in obtaining necessary services for basic necessities.
- Assists clients in accessing self-help support groups and increasing social support networks.
- Provides appropriate interventions to aid clients in the restoration of daily living skills.
- Coordinates and assists in crisis management and stabilization.
- Collaborates and communicates effectively with the treatment team for the coordination of care.
Qualifications
- High School diploma plus 3 years of direct experience.
- Associates or Bachelor’s degree in a social sciences field or related field is preferred.
- Spanish/English bilingual.
- An understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior.
- An understanding of how to therapeutically engage a person with mental illness.
- Valid driver’s license, car insurance, and insurable driving record required.
- Willingness to travel throughout the assigned service area.
- Must have reliable transportation.
- An understanding of the community mental health system.
