Bilingual Case Manager

We are seeking a Targeted Case Manager to deliver quality case management services for children, families, and adults. Key Responsibilities Include Developing personalized service plans with input from the treatment team and Clinical Supervisor.Connecting clients and families with community resources and following up on referral success.Ensuring all documentation complies with payer, state, and accreditation standards within designated timeframes.Maintaining regular contact with clients through weekly or bi-weekly meetings for up to 12 months.Participating in bi-weekly treatment team meetings and acting as a liaison with other professionals and agencies.Recruting new referrals to maintain caseloads and providing 24/7 on-call support as required.Ensuring compliance with all relevant regulatory bodies. Qualifications For Mississippi: Master’s degree in human services, behavioral health, social work, counseling, or related field; 2+ years of mental health experience; credentials such as LSW, DMH-approved therapist, or RN are required.For Florida: Bachelor’s degree in a human services discipline; experience with children and families in case management or community resources.
