BH Case Management Team Lead

Job Summary

The Behavioral Health Case Management Team Lead will manage CHC members with behavioral health diagnosis through comprehensive evaluation and resource coordination. They will participate as a team member in CHC Behavioral Health Department, collaborating with utilization management, member services, and quality improvement departments.

Qualifications

• Master's Degree in Social Work, Counseling required.
• LMSW, LCSW, LPC, LMFT required.
• Certified Case Manager (CCM) or Associate Certified Manager (ACM) preferred.
• Minimum of 4 years BH Case Management experience, including at least 1 year in an MCO setting.

Work Experience

Minimum of 4-years BH Case Management to include Minimum of 1 Year MCO experience.

Software Proficiencies

• Microsoft Office (Word, Outlook, Excel)
