About the role
We are seeking a full-time team lead to join our mission-driven organization where mental health meets independence. The position involves maintaining a caseload and overseeing a team helping individuals with mental health and/or substance use diagnoses. It offers the chance to help people regain independence and change the trajectory of lives.
Responsibilities
- Maintain a caseload and oversee the team helping individuals with mental health and/or substance use diagnoses.
- Provide and document a minimum of 30 hours of billable services to clients per week.
- Complete all opening paperwork for all new clients.
- Review state outcomes quarterly.
- Review FARS/FASAMS assessments and service plans at their due date or sooner if appropriate.
- Complete initial leases and income certifications as well as their reviews annually.
- Complete all documentation on client services no later than 24 hours after the service has been provided.
- Comply with all company policies and procedures and all contract requirements.
- Communicate regularly with supervisor to ensure expectations are met for funding, staffing, client issues, etc.
- Maintain appropriate boundaries in the workplace and work professionally (ethically), especially regarding confidentiality.
- Interact professionally with co-workers, residents, and the community.
Requirements
- Candidates must possess a Bachelor's Degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field and have a minimum of three years of full time or equivalent experience working with adults with severe and persistent mental illness, OR
- A Bachelor's Degree from an accredited university or college and five years, full time or equivalent, working with adults with persistent mental health diagnosis.
- One-year supervisory experience is preferred.
- Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population.
- Must have knowledge of available resources for adults with severe and persistent mental illness.
- Must have knowledge of the principles and practices of case management.
- Demonstrates above average verbal and written skills.
- Demonstrates efficient computer skills (40 wpm).
- Demonstrates excellent electronic health records and ability to effectively train.
- Demonstrates skilled knowledge and use of Microsoft Suite.
- Be able to work with diverse groups and individuals.
- Successfully complete all criminal background screens and remain eligible.
