Description / Position Overview
In this (special, at-will) grant-funded position, you will play a critical role in the CPH (Community and Public Health) division of the CSFD (Colorado Springs Fire Department).
You will be responsible for performing assessments, intensive case management, behavior modification, and solution focused treatment for members of our CPH programs.

We are currently hiring for the TAP Team (Transitional Assistance Program). In this role you will be part of an intensive navigation team assisting individuals exiting the criminal justice system and reintegrating back into our community.
This is a grant funded position with funding through June 2027 with the possibility of extension.

Normal work hours are Monday through Thursday, 8:00 a.m. to 6:30 p.m. OR Tuesday through Friday, 8:00 a.m. to 6:30 p.m.

As a Behavioral Health Clinical Navigator, You Will
- Conduct intensive case management and community navigation for members on a daily basis;
- Manage a caseload of high-needs clients;
- Complete behavioral health intake/assessment for those individuals referred to CPH programs;
- Conduct intakes/interventions in homes and other public and semi-public locations;
- Coordinate care for CPH members with primary care, behavioral health, and community partners;
- Implement behavioral modification/solution focused treatment for issues that are hindering improvement;
- Create intervention planning/goal setting for each member assigned;
- Participate in case consultation, internal and external, to assist compliance of member;
- Interact in professional-to-professional communication to advocate for member;
- Complete case conceptualization/differential diagnosis/presentation in case review, internal staff meetings, and community meetings;
- Represent CPH at community meetings and report outcomes to supervisor;
- Assist with referral management, input, and acquisition;
- Provide community training and education;
- Assist with community outreach, building relationships with partners/agencies, and maintaining positive alliance;
- Monitor emergency medical response interactions and notifications for compliance
Knowledge Of
- We are looking for candidates who demonstrate:
- Appropriate scope of practice expectations;
- The DSM-V and Cognitive Behavioral Therapy/Solution focused treatment/ Systems model;
- High needs, severely mentally ill clientele;
- Substance abuse and polypharmacy concerns/treatments;
- Goal setting and treatment planning;
Ability to:
- Complete assigned tasks with minimal supervision;
- Demonstrate leadership and task management skills;
- Formulate cohesive intervention plans;
- Navigate electronic medical records independently after training;
- Set clear and firm boundaries with clients;
- Function in a complex and fluid work environment;
- Communicate professionally, effectively, and efficiently with all stakeholders;
Minimum Qualifications
- A master's degree from an accredited college or university with major coursework in social work, counseling, psychology, or related field;
- One year of full-time experience working with patients in a behavioral health setting, including internship;
- Possess, or obtain upon hire, and maintain a valid, non-probationary Colorado driver's license not subject to restriction related to alcohol and/or drug violations or pending charges;
- Registered psychotherapist or candidate with DORA (Department of Regulatory Agencies);
Preferred Qualifications
- Experience working with individuals involved in the criminal justice system;
- Experience working with individuals with behavioral health and substance use disorders;

- The background check process for this position includes being fingerprinted by the Colorado Springs Police Department.
