Behavioral Health Patient Navigator

Responsibilities

  • Assess patients for social, behavioral health, and practical support needs
  • Absorb patients with accessing community resources and services
  • Coordinate care between providers and outside organizations
  • Conduct outreach calls to schedule, confirm, and follow up on appointments
  • Support patient engagement and retention in treatment
  • Document interactions and patient needs within the EMR
  • Work collaboratively with therapists, psychiatrists, medical providers, and administrative staff
  • Assist with referrals, transportation coordination, insurance issues, and other patient support needs

Qualifications

  • A high school diploma or equivalent is required
  • An associate’s or bachelor’s degree is preferred but not required
  • Experience in healthcare, behavioral health, community outreach, care coordination, or customer service is strongly preferred
  • Strong communication and organizational skills are essential
  • The candidate should be comfortable speaking with patients over the phone and in person
  • Bilingual candidates are encouraged to apply
  • Experience working with Medicaid or underserved populations is a plus
