Behavioral Health Patient Navigator

Responsibilities
- Assess patients for social, behavioral health, and practical support needs
- Absorb patients with accessing community resources and services
- Coordinate care between providers and outside organizations
- Conduct outreach calls to schedule, confirm, and follow up on appointments
- Support patient engagement and retention in treatment
- Document interactions and patient needs within the EMR
- Work collaboratively with therapists, psychiatrists, medical providers, and administrative staff
- Assist with referrals, transportation coordination, insurance issues, and other patient support needs
Qualifications
- A high school diploma or equivalent is required
- An associate’s or bachelor’s degree is preferred but not required
- Experience in healthcare, behavioral health, community outreach, care coordination, or customer service is strongly preferred
- Strong communication and organizational skills are essential
- The candidate should be comfortable speaking with patients over the phone and in person
- Bilingual candidates are encouraged to apply
- Experience working with Medicaid or underserved populations is a plus
