Accountabilities

The Bed Readiness RN Referral Coordinator is responsible for managing referral workflows, supporting patient placement decisions, and ensuring effective coordination between clinical teams and post-acute care partners. This role requires strong clinical judgment, communication skills, and the ability to manage complex placement challenges while maintaining compliance and patient-focused standards.

  • Coordinate incoming and outgoing referrals for post-acute placements, ensuring accurate, complete, and timely processing.
  • Review patient clinical information and collaborate with interdisciplinary teams to determine appropriate care levels and placement options.
  • Act as the primary liaison between hospital teams, internal programs, and external care facilities to facilitate efficient communication and transitions.
  • Monitor bed availability, capacity, and placement timelines to optimize patient throughput and reduce avoidable discharge delays.
  • Track referral activity, turnaround times, and placement barriers to identify opportunities for workflow improvements.
  • Ensure adherence to regulatory requirements, payer guidelines, and organizational policies related to care transitions and referrals.
  • Support escalation and resolution of complex placement issues by partnering with leadership and external organizations.
  • Promote safe, efficient, and patient-centered transitions throughout the care continuum.

Requirements

The ideal candidate is an experienced nursing professional with strong care coordination skills and the ability to balance clinical priorities with operational needs. Success in this role requires excellent communication, organization, and collaboration skills, along with experience supporting patient care transitions.

  • Bachelor's degree in Nursing, or completion within five years of initial employment; equivalent consideration may apply for eligible experienced internal candidates.
  • Current Registered Nurse license in Washington State or a multistate license endorsement.
  • Current Basic Life Support (BLS) certification required.
  • Advanced nursing degree (MSN, PhD, DNP, ARNP) preferred.
  • Specialty certification preferred.
  • Additional certifications such as ACLS, NALS, PALS, TNCC, or NRP may be required depending on department needs.
  • Minimum one year of acute care experience preferred.
  • Strong understanding of post-acute care coordination, referral management, and discharge planning processes.
  • Ability to communicate effectively with healthcare professionals, patients, families, and external partners.
  • Strong organizational skills with the ability to manage multiple priorities and complex cases.
