About the role

As a Licensed Practical Nurse (LPN) Supervisor at Woodlands Village, you will play a crucial role in enriching the lives of seniors by providing compassionate, high-quality nursing care. Your responsibilities include conducting assessments, administering medications, coordinating services, and collaborating with interdisciplinary teams to promote optimal resident outcomes.

Responsibilities

  • Provide nursing care to residents in accordance with established care plans, policies, and procedures.
  • Conduct ongoing assessments of residents' physical and psychosocial status and monitor for changes in condition.
  • Administer medications and treatments as prescribed and document appropriately.
  • Cookordination and communication of resident care needs with physicians, families, pharmacies, and other care providers.
  • Support health promotion, disease prevention, and wellness initiatives within the community.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Aid in care planning, service plan updates, and resident evaluations as required.
  • Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
  • Support training and guidance for care staff as appropriate.
  • Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
  • Participate in quality assurance initiatives, audits, and compliance activities as needed.
  • Perform other duties as assigned.

Qualifications

  • Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations.
  • Minimum of two (2) years of Licensed Nurse experience required.
  • Experience in assisted living, long-term care, or a skilled nursing environment preferred.
  • Strong clinical judgment, problem-solving, and decision-making skills.
  • Effective organizational, time management, and communication skills.
  • Demonstrated commitment to seniors and person-centered care.
  • Ability to manage multiple priorities and delegate appropriately.
  • Ability to work nights, weekends, and flexible schedules as required by the community.
