Responsibilities

  • Carry out direct care duties as assigned, including encouraging independence, treating residents with dignity and respect, and assisting with meal preparation and recreational activities.
  • Ensure supervision of assigned Direct Support Professionals, providing instruction, modeling, observing, and maintaining up-to-date assignment sheets.
  • Coordinate shifts, ensure active programming completion, and document resident and facility needs.
  • Assist with meal-time processes, adhere to dietary needs, and ensure proper use of communication systems.
  • Inform managers of events, incidents, and serious reportable events, and ensure all paperwork and documentation are complete.
  • Attend meetings and provide in-service training on goal implementation, documentation, and related areas.
  • Ensure assigned direct care staff completes required training and participate in the on-call system.
  • Maintain the house in a clean, safe, and organized manner, monitor supplies/equipment, and inform the manager of needs.
  • Adhere to Agency policies and procedures, including relevant regulations.

Requirements

  • High school diploma or equivalent required.
  • Associates degree in the human service field, with at least one-year experience with individuals with I/DD preferred.
  • Must be at least 18 years of age.
  • Must meet applicable Vehicle Operator Requirements (NYS Driver’s License Required).
  • Must adhere to CDS Life Transitions, Inc. policies and procedures.
  • Must be able to stand, walk, stoop frequently, push/pull repeatedly, lift at least 50 lbs. dead weight.

  • Ability to work independently and motivate others.
  • Effective communication skills, both oral and written.
  • Able to cover shifts as needed.

Skills

  • Knowledge of human service field.
  • Experience working with individuals with intellectual and developmental disabilities.
  • Ability to follow IPPs and ensure supervision of staff.
  • Proficiency in communication systems and documentation.
  • Ability to coordinate schedules and ensure active programming.
  • Strong organizational and time management skills.
  • Ability to maintain a clean, safe, and organized living environment.
  • Compliance with relevant regulations and policies.

The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting. The offered salary will be determined by: Applicant qualifications and experience, education, position specific licensing/training and departmental budgets.

Schedule

Shifts may vary based on the needs of the program.
