About the Program
The Commonwealth of Massachusetts Executive Office of Health and Human Services (EOHHS), through the MassHealth Office of Long-Term Services and Supports (LTSS), provides a robust system of care for individuals of all ages with disabilities and chronic conditions. The program is designed to help Massachusetts residents live with independence and dignity, participate in their communities, and improve their overall quality of life. As the Public Assessors Option (PAO), Maximus provides education to providers, conducts comprehensive in-person assessments, and offers call center support for the following LTSS programs: Adult Day Health (ADH), Adult Foster Care (AFC), Group Adult Foster Care (GAFC), and Personal Care Attendant (PCA). As the PAO, Maximus serves as a point of entry for LTSS inquiries and initial contacts while conducting timely, high-quality, consistent, and accurate assessments to determine clinical eligibility for these programs.
Role Overview
The Assessment Coordinator serves as a key member of the MassHealth Public Assessor Option (PAO) team, providing comprehensive support to individuals seeking Long-Term Services and Supports (LTSS) through programs such as Adult Day Health (ADH), Adult Foster Care (AFC), Group Adult Foster Care (GAFC), and Personal Care Attendant (PCA). The Assessment Coordinator is responsible for managing the intake and assessment preparation process, conducting telephonic screenings, coordinating required documentation, educating members on available program options, and facilitating timely scheduling of assessments. This role serves as the primary point of contact for members once an assessment request has been initiated and plays a critical role in ensuring a smooth, person-centered experience. The Assessment Coordinators are cross-trained to perform Customer Service Representative (CSR) functions and may provide front-line member support as operational needs require.
Responsibilities
- Review and validate data and reports.
- Work assigned reports on a regular basis to identify necessary fixes or work-arounds.
- Make recommendations to improve processes.
- Ensure assignments are current and include proper documentation.
- Use various tracking tools to record assignments, processes and defects.
- Review, research and audit system results.
Requirements
- High School diploma, GED, or equivalent required.
- 2-4 years of relevant experience required.
- Minimum 3 - 5 years of experience in healthcare, human services, care coordination, customer service, case management, or related field.
- Residency in the state of Massachusetts.
- Proficiency in Microsoft Office applications.
- Proficiency in computer skills, with the ability to navigate multiple systems and applications efficiently.
- Strong ability to multitask and manage competing priorities effectively.
- Strong verbal and written communication skills.
- Excellent organizational and follow-up skills.
- Strong attention to detail and documentation accuracy.
- Valid driver's license and reliable transportation for minimal field travel.
Preferred Qualifications
- Associate or Bachelor's degree in business, social work, healthcare or relevant field.
- Experience supporting LTSS, Medicaid waiver programs, managed care, or healthcare assessments.
- Knowledge of MassHealth programs and eligibility requirements.
- Experience coordinating documentation between members, providers, and healthcare organizations.
- Fluency in both Spanish and English, including the ability to speak, read, and translate effectively.
Work Environment
This is a primarily remote position; however, occasional field-based work may be required within the assigned geographic service area. Candidates must have a valid driver's license and reliable transportation. Eligible business travel is reimbursed at the current IRS mileage rate, currently $0.76 per mile.
