APP Fellow (Nurse Practitioner or Physician Assistant) - Adult Critical Care

About the Role

This one-year Advanced Practice Provider (APP) Fellowship is designed to provide clinical experiences and a robust didactic curriculum, preparing APPs for full-time positions in specialty areas, with a focus on critical care. The fellowship is ideal for new APPs or those wishing to change specialties.

The APP Fellow will rotate through multiple critical care settings, including Neuroscience, Cardiothoracic, Medical/Surgical, Specialty Medical, Cardiac, and Surgical/Trauma/Burn ICUs, with additional elective rotation opportunities. The fellowship includes training in critical care procedures such as central venous catheters, arterial catheters, lumbar punctures, and more.

Fellows will participate in weekly lectures, grand rounds, and complete an end-of-year project. The schedule aligns with department APPs but may include additional clinical and academic requirements.

Responsibilities

  • Provides direct patient care, including taking comprehensive histories and performing physical examinations.
  • Evaluates and treats patients based on history, physical examination, and diagnostic test results.
  • Performs therapeutic procedures in accordance with procedural privileges and proficiency, including:
    • Lumbar puncture
    • Central venous line placement
    • Pulmonary artery catheterization
    • Arterial line placement
    • Endotracheal intubation
    • Chest tube placement
    • Bronchoscopy
    • Thoracentesis
    • Paracentesis
    • Suturing/stapling and removal of catheters/tubes/drains
  • Initiates referrals to other healthcare providers and consults with attending or collaborating physicians.
  • Documents and bills for direct care provided.
  • Utilizes current research and evidence-based decision-making in clinical practice.
  • Participates in quality/performance improvement activities and clinical research.
  • Attends education and academic sessions as determined by the APP Fellowship Coordinator or Director.
  • Completes presentations, lectures, QI projects, or other academic requirements.
  • Participates in required MUSC APP Fellowship activities.
  • Maintains licenses, certifications, CNEs/CMEs, and other requirements as per state law and organizational policies.

Requirements

  • Completion of an accredited Physician Assistant (PA) program and currently licensed or eligible for licensure as a PA, or completion of an accredited Nurse Practitioner (NP) program and currently licensed or eligible for licensure as an APRN (Adult Gerontology Acute Care NP or Acute Care NP).
  • No prior experience required; fellowship is geared toward new graduates or experienced APPs transitioning to critical care.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (must be current prior to start date).
  • DHEC and DEA license applications must be started prior to the first day if applicable.

Schedule

Fellowship duration: February 2027 through February 2028.

  • ICU rotations: 13-hour shifts, 3-4 clinical shifts per week, including nights, weekends, and holiday coverage.
  • Non-ICU rotations: 8-10 hour shifts, 4-5 clinical shifts per week.
  • Scheduled weekly hours: 40 hours, with additional didactic requirements.

Qualifications

Focus on intensive care for patients aged 13 years to death, including those who are acutely ill, injured, potentially unstable, or technologically dependent (e.g., ventilators, drips, monitors).

Supervision

APP Fellows will be assigned a Primary Supervising/Collaborating Physician employed by MUSC and will report directly to this physician, the APP Manager, and the APP Fellowship Director.

Physical Requirements

  • Continuous standing, walking, and sitting; ability to bend, kneel, climb stairs, and reach in all directions.
  • Full use of both hands and legs, good finger dexterity, and ability to perform repetitive motions.
  • Qualified physically for respirator use.
  • Maintain 20/40 corrected vision and work indoors in a latex-safe environment.
  • Frequently lift, lower, push, pull, or carry objects weighing up to 50 lbs unassisted; exert up to 50 lbs of force; lift from 36" to overhead up to 25 lbs.
  • Infrequent work in dusty areas and confined/cramped spaces.
