Responsibilities
- Collaborate with primary care and specialty providers to develop and implement care plans
- Absess patients' physical, psychosocial, and emotional needs
- Ensure adherence to clinical protocols and documentation standards
- Provide patient education, support, and utilize critical thinking to address individual needs
- Support providers in daily patient management and oversee clinical staff when necessary
Qualifications
- RN license with a Nursing Diploma/Associate's degree
- 2-5 years of experience
- Basic Life Support (BLS) certification
- PREFERRED: Bachelor's Degree in Nursing and 5-10 years of experience
