Aging Care Manager 3 (LG), Protective Services - Wayne County Area Agency on Aging

Located in Honesdale, PA, Wayne County Area Agency on Aging (AAA) is seeking a qualified Aging Care Manager 3 to join a dedicated team committed to enhancing the lives of older adults. Responsibilities

• Conduct and oversee complex investigations into alleged abuse, neglect, exploitation, or abandonment
• Assess the medical, social, psychological, financial, and environmental needs of people in high-risk and complex circumstances
• Develop, implement, and monitor service plans to reduce or eliminate risk and assist older adults with accessing needed resources
• Recommend and coordinate advanced protective services and referrals with internal and external organizations, care providers, and community organizations
• Recommend and coordinate court interventions and testify in legal proceedings
• Perform supervisory duties, provide guidance, review casework for quality and compliance, participate in training, and assist with other levels of care management when needed

Qualifications

• One year as an Aging Care Manager 2 or Aging Program Assessor; or
• One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience

Other Requirements:

• You must meet the PA residency requirement
• You must be able to perform essential job functions

Legal Requirements:

• You must pass a background investigation
• A conditional offer of employment will require a drug screening
• This position falls under the provisions of the Child Protective Services Law
• A conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance
