Aging Care Manager 2 (LG) Westmoreland County Area Agency on Aging

The Westmoreland County Area Agency on Aging seeks a full-time Aging Care Manager II to support vulnerable older adults by managing a caseload and addressing complex situations.

Responsibilities

• Caseload Management: Maintain a caseload of vulnerable consumers and use independent judgment to handle routine and complex cases
• Information Gathering: Collect details from consumers, families, neighbors, and others to identify social, economic, emotional, health, or physical concerns
• Investigation Work: Investigate Reports of Need involving potential abuse, neglect, abandonment, or financial exploitation
• Risk Evaluation: Review gathered information to identify risks and apply regulations to determine substantiation
• Protective Actions: Provide protective interventions such as court petitions, emergency relocation, or referrals for community services

Schedule

• Full-time, 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch
• May be required to be on a standby rotation for off-hours coverage
• Combines office and fieldwork; telework may be considered on a case-by-case basis after initial training and probationary period

Requirements

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training
• Must meet Pennsylvania residency requirement
• Must be able to perform essential job functions
