Aging Care Manager 3 (LG) - Allegheny County Area Agency on Aging, Protective Services
The Allegheny County Department of Human Services, Area Agency on Aging is expanding its Protective Services Unit. We are looking for robust and compassionate professionals to assist in protecting and empowering the older adults in our community. These roles perform protective services investigations, provide expert guidance during key processes, and contribute directly to our mission.
About the role
These positions support a caseload of vulnerable older adults who need protection from abuse, exploitation, abandonment, or self-neglect. Work involves conducting protective service investigations, providing technical support to staff and contractors, and other actions that rely on maintaining, updating, and applying extensive knowledge of Aging programs, services, and regulations.
Responsibilities
- Risk Evaluation: Receive and screen Reports of Need for Protective Services to determine level of urgency and ensure timely follow-up
- Protective Investigations: Conduct interviews, review detailed medical and financial records, and analyze information and evidence; determine, recommend, and follow through with actions needed to provide appropriate protection and support
- Case Coordination: Participate in case conferencing with law enforcement, legal, behavioral health, social service, and other professionals as needed to resolve challenging or complex cases
- Care Planning: Participate in care and service planning with individual older adults to reduce and eliminate risk; provide referrals to community organizations for follow-up or other case managers for on-going case supervision
- Technical Guidance: Provide expert support to staff and community partners regarding regulations and procedures
- Document Management: Maintain detailed, organized records, logs, and written reports to support case activity
Schedule
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch
- Hybrid work of up to two days per week after six months of employment (telework opportunity subject to change; requires securely configured high-speed internet and an approved location inside Pennsylvania)
Requirements
Minimum Experience and Training
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience, and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience
Other Requirements
- Must meet Pennsylvania residency requirement
- Hiring preference may be given to candidates who live within Allegheny County
- Must be able to perform essential job functions
- Must pass a background investigation, including PA State Police and FBI clearance under the Older Adult Protective Services Act
