About the role

The Advanced Practice Provider II (APP) provides direct patient care within Cottage Medical Group (CMG) and/or the hospital. Functions independently and collaboratively with supervising clinic physician(s) and leadership to deliver patient care. Assesses, interprets, and integrates findings to determine diagnostic, educational, and therapeutic management plans for patients.

Responsibilities

  • Performs medical services in clinic and hospital settings within scope of education, training, and experience:
    • Conducts patient visits per accepted practice standards.
    • Obtains patient histories and performs physical examinations.
    • Orders and evaluates laboratory and diagnostic studies.
    • Develops and implements treatment plans in accordance with clinical practice guidelines and protocols.
    • Monitors effectiveness of therapeutic interventions.
    • Assesses and manages patient follow-up care.
  • Collaborates with physicians to manage acute and long-term medical needs of patients (40%):
    • Provides consultations, monitoring, and continuity of care between clinic visits.
    • Performs pre- and post-treatment surveillance and post-op visits.
    • Maintains and reviews patient records, charts, and pertinent information.
    • Posts tests and examination results.
    • Triages patient telephone calls and provides consultation.
    • Seeks physician input on treatment and care questions.
  • Works with physicians and care teams to establish and manage Transitional Care and Chronic Care Management (30%).
  • Manages a sub-clinic to provide patient assessment, education, and clinical recommendations within scope of practice.
  • Prescribes pharmaceuticals, medications, and treatment regimens; refers patients to specialists as appropriate (10%).
  • Performs peripheral intravenous access consistent with training guidelines.
  • Demonstrates organizational and communication skills to coordinate services with co-workers and other departments (10%).
  • Responds to stressful situations calmly and professionally.
  • Facilitates education and professional development of patient care practitioners (10%):
    • Participates in education of clients, families, and the community.
    • Assists staff with maintaining clinical competencies and promoting critical thinking.
    • Develops clinical orientation frameworks for professional staff.
    • Mentors preceptors and professional nursing staff.
    • May supervise ancillary staff and perform research.

Requirements

  • Graduate of an accredited Physician Assistant program (Master’s degree or certificate from a specialized medical training program) or Master of Nursing Degree from an accredited Nurse Practitioner program approved by the California Board of Registered Nursing.
  • Current California licensure and certification as a Nurse Practitioner or Physician Assistant.
  • Approval for privileges by Medical Staff (if applicable).
  • American Heart Association (AHA) Basic Life Support (BLS) certification within 45 days of hire.
  • Two (2) years of PA or NP experience or two years of experience specific to the practice specialty in an outpatient and/or inpatient setting.
  • Knowledge of medical terminology and electronic medical records (EMR).
  • Experience with EPIC EMR preferred.
  • Bilingual in English/Spanish preferred.

Pay

Compensation for non-physician positions is determined based on related years of experience and internal equity. Eligible employees may receive additional compensation, including shift differentials, on-call pay, incentive pay, and bonuses. Manager and above positions may participate in Cottage Health’s annual management incentive program. Physician compensation is determined by specialty and may include bonus potential.
