Advanced Practice Provider - APP - Family Medicine
About the Role
SSM Health’s Waupun Clinic is a growing, collaborative outpatient Family Medicine practice located near the hospital campus in the scenic Rock River Valley. The position is a full‑time, 100% outpatient role where you will hold your own patient panel and practice at the top of your license, supported by patient‑focused staff and built‑in administrative time.

Minimum Qualifications
- Graduate of a nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) program
- NPs should be APRN
- Ability to obtain PA/NP licensure in Wisconsin
