About the role
Provides clinical leadership to enhance compliance with quality and care standards, managing inter-professional teams for individual or multiple cases. Acts as a resource for clinical and operational guidance, delivering advanced nursing care within regulatory and national standards in Manhattan.
Responsibilities
- Manage and deliver full scope of advanced nursing practice, evaluating patient responses and adjusting care plans to achieve optimal outcomes.
- Develop and revise clinical protocols in collaboration with physicians, providing training as needed.
- Conduct comprehensive assessments, including physical exams, history-taking, screenings, interventions, treatments, referrals, discharge planning, and patient education.
- Collaborate with patients, families, and healthcare teams to provide continuous, coordinated care across settings.
- Ensure clinical services comply with patient-centered standards and quality measures like HEDIS and QARR.
Requirements
- Licensed Registered Nurse and Nurse Practitioner in New York, with specialty certification (adult, family, gerontology, or psychiatric).
- Master’s Degree in Nursing from an accredited program; additional psychiatric-mental health certification for psychiatric NPs.
- Minimum two years of practice as a nurse practitioner, with experience in home care or management preferred.
- Knowledge of quality measures, coding, and bilingual skills may be required.
Pay
USD $109,900 - $146,500 annually.
