About the Role

Provides clinical leadership to enhance quality and compliance standards. Manages inter-professional teams delivering comprehensive nursing care within regulatory guidelines. Acts as a resource for clinical and operational support to achieve team outcomes. Works under supervision to evaluate patient responses, develop protocols, and coordinate care across settings. Ensures adherence to Patient-Centered Medical Home standards and quality measures.

Responsibilities

  • Perform physical and psychological assessments
  • Develop and implement treatment plans
  • Provide patient education and care management
  • Collaborate with patients, families, and healthcare providers to optimize care delivery

Requirements

  • Valid NY State RN license
  • Nurse Practitioner certification in adult, family, or gerontology health
  • Maintain current credentials, including NPI, Medicaid, and Medicare numbers
  • Psychiatric Nurse Practitioners: PMHNP-BC certification
  • Knowledge of quality measures and coding

Qualifications

  • Master’s degree in nursing (preferred)
  • 2+ years of practice experience (preferred)
  • Bilingual skills (advantageous)
  • Clinical home care or management experience (advantageous)
