About the Role
Provides clinical leadership to enhance quality and compliance standards. Manages inter-professional teams delivering comprehensive nursing care within regulatory guidelines. Acts as a resource for clinical and operational support to achieve team outcomes. Works under supervision to evaluate patient responses, develop protocols, and coordinate care across settings. Ensures adherence to Patient-Centered Medical Home standards and quality measures.
Responsibilities
- Perform physical and psychological assessments
- Develop and implement treatment plans
- Provide patient education and care management
- Collaborate with patients, families, and healthcare providers to optimize care delivery
Requirements
- Valid NY State RN license
- Nurse Practitioner certification in adult, family, or gerontology health
- Maintain current credentials, including NPI, Medicaid, and Medicare numbers
- Psychiatric Nurse Practitioners: PMHNP-BC certification
- Knowledge of quality measures and coding
Qualifications
- Master’s degree in nursing (preferred)
- 2+ years of practice experience (preferred)
- Bilingual skills (advantageous)
- Clinical home care or management experience (advantageous)
