Advanced Practice Clinician - Full Time

Overview

Provides clinical leadership to enhance compliance with quality and cost standards, managing inter-professional teams and delivering advanced nursing care in accordance with regulations and standards. Acts as a key resource for clinical and operational guidance, supporting team outcomes.

Responsibilities

  • Managing and delivering full-scope advanced nursing practice for targeted populations, evaluating responses, and updating care plans for optimal outcomes.
  • Collaborating with physicians to develop and train staff on clinical protocols.
  • Providing comprehensive care including physical exams, history, screening, interventions, treatments, referrals, and patient education.
  • Working with patients, families, and team members to assess, plan, and provide continuous care management.
  • Ensuring care compliance with Patient-Centered Medical Home standards, meaningful EHR use, and quality measures like HEDIS and QARR.

Qualifications & Skills

  • Valid RN license and NP certification in New York, with specialization in adult, family, or gerontology health.
  • Current ANCC or AANP certification; psychiatric NPs require PMHNP-BC certification.
  • Master's degree in Nursing from an accredited program; clinical experience with a minimum of two years as a nurse practitioner preferred.
  • Knowledge of coding, quality measures, and bilingual skills may be required.
  • This role involves clinical leadership, comprehensive patient care, protocol development, and team collaboration to improve health outcomes in a home and community setting.
