About the role
Provides clinical leadership to enhance compliance with quality and care standards, managing service delivery for inter-professional teams. Acts as a key resource for clinical and operational guidance to achieve team outcomes. Delivers advanced nursing care following state, federal, and national standards, working under the supervision of the Clinical Director. This role involves evaluating patient responses, developing and revising care protocols, and collaborating with patients, families, and healthcare providers to ensure comprehensive care. Ensures services align with Patient-Centered Medical Home standards and quality measures such as HEDIS and QARR.
Responsibilities
- Manage full scope of advanced nursing practice, assessing patient responses and adjusting care plans
- Develop and train staff on clinical protocols
- Provide comprehensive clinical services including examinations, interventions, treatments, and patient education
- Coordinate care with patients, families, and healthcare teams, ensuring continuous and effective management
- Maintain compliance with healthcare standards and quality measures
Qualifications
- Valid NY State RN license and Nurse Practitioner certification in adult, family, or gerontology health
- Master’s degree in Nursing from an accredited program
- Certification from ANCC or equivalent; psychiatric NP certification if applicable
- Minimum two years of NP experience preferred, with clinical home care or managerial experience advantageous
- Knowledge of coding, quality measures, and bilingual skills as needed
