About the role
Provides clinical leadership to enhance compliance with quality and care standards, managing inter-professional teams for individual or population-based cases. Acts as a key resource, offering clinical and operational guidance to achieve team outcomes. Delivers advanced nursing care in accordance with state, federal, and national standards, working under the supervision of the Clinical Director for the Nurse Practitioner Program. This role covers Manhattan.
Responsibilities
- Managing and delivering full scope of advanced nursing care, evaluating patient responses, and revising care plans to ensure quality outcomes.
- Collaborating with physicians to develop and train staff on clinical protocols.
- Providing comprehensive care, including assessments, interventions, treatment planning, referrals, and patient education.
- Coordinating with patients and healthcare teams to deliver continuous, patient-centered care across settings.
- Ensuring compliance with standards such as Patient-Centered Medical Home, meaningful use, and quality measures like HEDIS and QARR.
Qualifications
- Licensed Registered Nurse and Nurse Practitioner in New York, with specialty certification (adult, family, gerontology, or psychiatric-mental health).
- Master’s degree in Nursing with current certification from ANCC or AANP.
- Minimum two years of nurse practitioner experience preferred, with clinical home care or managerial experience advantageous.
- Knowledge of quality measures, coding, and bilingual skills as required.
