About the role
Full-time position offering flexibility and a hybrid work environment (home, community, office setting). We are seeking Care Coordinators to join our team in the Augusta, Skowhegan, and Richmond areas.
Responsibilities
- Develop and implement individualized care plans for clients.
- Educate clients on health and wellness topics.
- Write the Plan of Care for each member.
- Identify and link members with community resources.
- Facilitate referrals and respond to social service needs.
- Collaborate with other team members including doctors, nurses, other care coordinators, and clinicians.
- Help members implement the Plan of Care.
- Monitor and assist members in problem-solving potential issues related to the healthcare system, financial, or social barriers.
- Support and encourage members in actively participating in reaching their goals.
Requirements
- Excellent communication skills for effective interaction with clients and healthcare professionals.
- Ability to work collaboratively within a team environment.
- Organizational skills to manage multiple client cases efficiently.
- MHRT/C, MHRT/Cp certification, or at least 1 year working as a Peer Support, Recovery Coach, or in the Daily Living Skills field, or a bachelor’s degree and experience working in mental health.
One year of experience in mental health work (MHRT-1) is preferred.
