Responsibilities

• Provide comprehensive access navigation services to patients and their families
• Coordinate with healthcare providers to ensure seamless patient care
• Facilitate access to community resources and support services
• Ensure compliance with all regulatory requirements and policies

Requirements

• Bachelor's degree in a related field
• At least 2 years of experience in healthcare navigation or related field
• Strong interpersonal and communication skills
• Proficiency in Microsoft Office Suite
