About the role

The Youth Transition Coordinator (YTC) partners with youth experiencing homelessness as they work to achieve safety, stability, and permanency. YTCs are skilled in quickly establishing rapport with youth—collaborating with them to develop a plan of care consisting of clear, measurable goals that address housing needs while also strengthening family relationships, addressing mental and/or physical health needs, educational needs, and employment opportunities.

The YTC plays an active role in engaging houseless and runaway youth in conversations focused on harm reduction, healthy relationships, overall support, and advocacy. YTCs participate in a variety of diversion activities, including family mediation and advocacy work within their assigned communities.

Requirements

  • Bachelor's degree with a minimum of one year's experience working in case management.
  • Must be able to pass background checks including DMV, CPS, APS, and criminal checks.
