Responsibilities
- Conduct weekly face to face mentoring sessions with participants.
- Identify high risk behaviors in youth and provide mentoring services to intervene in continued high risk activity.
- Participate in case conference meetings as scheduled and work collaboratively with case manager, therapeutic counselor, court staff and community resources working with participant.
- Respond to participant emergency notifications.
- Document all their daily activities, including but not limited to contact with program participants, conflict mediation encounters, and community engagement events.
- Risk-Reduction Planning - to modify high-risk behaviors.
- Attend case management meetings with when needed to support participants in goal accomplishment.
- Serve as linkages and support for individuals to enhance their assistance and use of opportunities and programs in the community (e.g. job programs, GED, drug treatment, and mentoring).
- Absorb other tasks and responsibilities as assigned.
Qualifications
- A High School Diploma or equivalent required.
- Credible Messenger/Community Engagement/Lived Experience 2+ years of experience working with at risk populations.
- Proficient computer skill with MS Office.
- Excellent verbal and written communication skills.
- Valid and clean driver's license required.

Salary: $27.24
Location: Hempstead
Schedule: Full-time, 35 hours per week. Mon-Fri evenings and weekends as needed.
