Youth Housing Case Manager Duties & Responsibilities
- Maintain a high level of supervision of youth in care, ensuring safety at all times and helping youth adhere to program guidelines and expectations.
- Develop and manage individualized case plans for youth residents in the Transitional Living Program through ongoing meetings and check-ins.
- Facilitate and coordinate communication between clients, family members, providers, and other agencies.
- Make appropriate referrals for medical, dental, health, and substance use needs.
- De-escalate and provide crisis intervention with clients.
- Aid clients in accessing services and resources.
- Maintain accurate and up-to-date client records.
- Follow mandated reporter requirements through documentation of incidents and calling CPS appropriately.
- Ensure that clients are following program guidelines.
- Collaborate with fellow staff and partners regarding youth needs and case plan follow-through.
- Attend community meetings and direct wrap-around meetings with staff and youth.
- Transport youth to appointments as needed.
- Participate in on-call rotation.
Requirements
- Experience/Education Requirements:
- AA + 90 quarter credits/60 semester credits + 2 years full-time (40hrs a week) direct experience with at-risk youth (lived exp = 1yr)
- Bachelor's degree + 1-year full-time (40hrs a week) direct experience with at-risk youth (lived exp = 6mos)
- 3-years full-time (40hrs a week) direct experience with at-risk youth (lived exp = 1yr)
- Skills Needed To Be Successful In This Role:
- Ability to record and document information on forms as required for case planning and CPS reporting.
- Ability to assess potentially dangerous situations, deescalate, and access emergency services if needed.
- Ability to build rapport and relate to youth and other staff while maintaining professional boundaries.
- Acceptance of a variety of lifestyles, behaviors, and cultural and spiritual practices along with a commitment to the agency’s mission of equity, diversity and inclusion work.
- Skills and ability to follow policies and procedures, maintain appropriate and professional boundaries.
- Attention to detail and ability to track and input data and complete all necessary paperwork.
- General Requirements:
- General High school diploma/GED or high school equivalency.
- Must be 21 years of age or older.
- 6 months minimum of experience working with youth.
- DCYF background clearance.
- Possess a valid driver's license, auto insurance, and reliable transportation.
- Physical Ability: Effective communication, hearing, reading, comprehending, operating phones, computers, and recording information for files and logs; ability to lift up to 50 Ibs; ability to run up and down indoor and outdoor stairwells in case of youth emergency; go up and down stairs.
