About the role
The Youth Coordinated Entry Case Manager is responsible for coordinating entry services for youth in need of support.
Responsibilities
- Assessing and referring youth to appropriate services based on their needs.
- Collaborating with community partners to ensure comprehensive support for youth.
- Maintaining accurate records and reporting on service outcomes.
Requirements
- Bachelor’s degree in social work, psychology, or a related field.
- At least 2 years of relevant experience in case management or social services.
- Strong interpersonal and communication skills.
- Proficiency in Microsoft Office Suite.
Qualifications
- Valid driver’s license and access to reliable transportation.
- Ability to work flexible hours including evenings and weekends.
