About the role

The Youth Care Coordinator provides targeted support services with youth-centered Systems of Care (SOC). SOC coordinates mental health and community services with the needs of children and families impacted by trauma, preventing out-of-home placements and surrounding families with support and resources.

Responsibilities

  • Case management
  • Individual and group rehabilitation
  • Wellness and care coordination

Requirements

  • Bachelor’s degree in social or human sciences from an accredited university
  • One-year experience working with juveniles in the mental health field or related social service field
  • Specialized training and/or certification in mental health services or related field as needed for providing adjunctive services, such as Juvenile Case Management Certification
  • Ability to see, hear, and speak clearly to interact with staff, consumers, family members, administration, and the community, both in person and on the telephone
  • Ability to work well under pressure, meet deadlines with efficiency and consistency, maintain a balance between good physical and mental health, and adhere to job requirements
  • Must be able to climb stairs and access non-handicapped equipped residences/facilities
  • Must possess a valid Driver's License and use personal automobile to travel to locations other than the primary office

Qualifications

  • Driver's License required
  • Mileage reimbursement provided for personal automobile use

Skills

  • Strong communication skills
  • Ability to work collaboratively in a multi-disciplinary team setting
  • Knowledge of trauma-informed care principles
  • Experience with case management and mental health services

Family & Children’s Services operates under the Certified Community Behavioral Health Clinic (CCBHC) model, which emphasizes an innovative, team-based approach to whole-person care for adults and children with complex mental health and substance use challenges. This model includes:

  • Innovative, team-based approach to care
  • Enhanced access to care and crisis services
  • Expanded traditional community mental health and substance use services
  • Care coordination and physical health screenings for mental health clients
  • Increased access to social services for clients' economic and social needs
  • Specialized services for veterans, those most in need, and those impacted by the opioid crisis
