Youth Assessment Center - Case Manager - Full - Time

About the role

The Youth Assessment Center seeks a dedicated Case Manager to provide comprehensive support to young individuals in need. This role involves assessing needs, developing plans, and coordinating services to ensure positive outcomes.

Responsibilities

• Conduct assessments to understand client needs
• Develop and implement individualized service plans
• Coordinate with other professionals to provide holistic care
• Maintain accurate records and reports
• Engage with clients and families to build trust and rapport
• Participate in regular team meetings to discuss progress and challenges

Requirements

• Bachelor’s degree in Social Work, Psychology, or related field
• At least 2 years of relevant experience in case management or social work
• Licenses/certifications in relevant fields (e.g., LMSW, LPC)
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Pay

$X per hour, based on experience and qualifications

Schedule

Full-time position, Monday through Friday, with occasional evening and weekend work as needed
