Young Adult Case Manager
Position Title and Overview
The 412 Youth Zone is located in downtown Pittsburgh and provides comprehensive, welcoming, and youth-driven services and support to young adults transitioning out of the foster care system or experiencing unstable housing. The Young Adult Case Manager develops and maintains strong, trusting relationships with young people to foster engagement and support their individual growth. They conduct regular meetings with young people in their homes, community settings, or at the drop-in center to evaluate their needs, address challenges, and celebrate successes. They collaborate with community partners and service providers to coordinate care, maximize support, and ensure young adults have access to necessary resources. They maintain accurate and timely documentation to ensure effective tracking and reporting. They facilitate and promote various programs occurring at the drop-in center to engage young people and meet their diverse needs. They teach young people how to gain and master adulting skills such as budgeting, home maintenance, and decision-making.
Location and Travel Requirements
- This position is primarily located in Downtown Pittsburgh.
- All positions are also expected to come to training, meetings, and other events at Main Campus in McKeesport, as scheduled.
- The travel required for this position is regular.
- This position requires frequent travel within the community, including visits to young people's homes, meeting in community settings, and transporting young people to appointments or other essential destinations.
Position Requirements
- Act 33/34 and FBI Clearances required.
- Bachelor's degree preferred, HS diploma/GED required with experience working with youth.
- Special consideration will be given to individuals with previous DHS system involvement as a client.
- Other combinations of education and experience will be considered.
- Valid PA driver's license, vehicle, registration, and proof of insurance.
- Requires travel to youth homes and throughout the community.
