Case Manager

Central Wyoming Counseling Center is seeking a compassionate, empathetic, and caring individual to join our team as a Case Manager.

About the Role

Provides non-clinical services assisting families by monitoring and following up, linking, referral, advocacy, and crisis intervention.

Schedule

Monday–Friday, 8:00 AM–5:00 PM (alternate schedule of 11:00 AM–7:00 PM may be available).

Responsibilities

• Conducts thorough and comprehensive case management assessments that identify the individual strengths and needs of each client.
• Synthesizes assessment information to prioritize problems that can be addressed by available services and resources.
• Coordinates and collaborates with clients and the therapist to develop personalized, achievable goals, objectives, and interventions that promote self-reliance.
• May perform other related duties as assigned.

Requirements

• Bachelor’s degree in a human-relations field (required).
• Minimum of one year of professional-level experience in human relations or a related field (preferred).
• Completion of a pre-employment drug screen.
• Completion of a background check.
• Satisfactory driving record.
