Case Manager

Essential Responsibilities

• Complete psychosocial assessments and maintain resident social histories.
• Cookordination discharge planning, including communication with hospitals, families, physicians, insurance providers, and community agencies.
• Assist residents and families with discharge needs, including home health, hospice, durable medical equipment, transportation, and community resources.
• Provide education regarding Medicare, Medicaid, insurance benefits, advance directives, and available financial assistance programs.
• Participate in interdisciplinary care conferences, resident care planning, and MDS meetings.
• Advocate for resident rights and assist in resolving resident and family concerns.
• Cookordinate referrals to community resources and support services.
• Maintain accurate and timely documentation in PointClickCare and resident medical records.
• Cookoperate with nursing, therapy, admissions, dietary, activities, and other departments to ensure continuity of care.
• Monitor discharge goals and barriers, proactively working to facilitate safe and timely transitions.
• Assist with regulatory compliance, survey readiness, and quality improvement initiatives.
• Maintain knowledge of federal, state, and facility regulations related to resident care and discharge planning.
• Perform other duties as assigned.

Qualifications

• Minimum Qualifications: Bachelor's degree in Social Work or a related human services field preferred. Minimum one (1) year of experience in skilled nursing, long-term care, or healthcare case management. Experience with discharge planning and community resources preferred. PointClickCare experience preferred. Knowledge of Medicare, Medicaid, resident rights, and long-term care regulations. Valid driver's license with an acceptable driving record if transportation or community visits are required.
