Wrap Facilitator - Social Services

About the role

The Wrap Facilitator manages a caseload within financial parameters, ensures timely completion of tasks, and develops and manages service delivery budgets. They conduct strengths-based assessments, collaborate with teams to develop plans of care, monitor progress, and adjust plans as needed. They also interpret evaluations, implement funder standards, and complete other duties as assigned.

Responsibilities

• Manages caseload within financial parameters

• Makes adjustments to plans based on progress
• Interprets evaluations
• Implements funder standards
• Completes other duties as assigned

Requirements

• Minimum bachelor’s degree in social work or related field
• Two years of relevant experience
• Experience with budget management
• Strong knowledge of mental health issues and commitment to empowerment
• Strong communication and writing skills
• Bilingual skills (Spanish preferred)
• CANS or similar assessment certification within 45 days of hire
• Valid driver’s license and auto insurance
• Demonstrated ability to work effectively with various stakeholders
